Directions to my Lynnwood Office (Main)
My Lynnwood office is conveniently located 15 miles from Seattle, 10 miles from Bothell, 15 miles from Everett and 20 miles from Marysville. The office is located on a main bus line as well as being less than a 15-minute walk from the Lynnwood Transit Center. The building complex provides free parking.

I-5 Northbound: Take Exit 181A, 44th Ave W exit toward WA-524 / Lynnwood. Turn left onto 44th Ave W (1st stoplight). Turn left onto 200th St SW (2nd stoplight). Turn left onto Cedar Valley Rd/52nd Ave W (3rd stoplight). 20102 Cedar Valley Rd is the 2nd building on the right. I am on the 1st floor in Suite 103. I-5 Southbound: Take Exit 181B toward WA-524 W exit. Turn slight right onto 196th St SW/WA-524. Turn left onto 50th Ave W (immediately past the 4th stoplight). 50th Ave W becomes Cedar Valley Rd/52nd Ave W (continue through 1st stoplight). 20102 Cedar Valley Rd is the 2nd building on the right. I am on the 1st floor in Suite 103. |
Directions to my Renton Office
My Renton office is conveniently located 14 miles from Seattle, 6 miles from SeaTac International Airport and 25 miles from Tacoma. In addition, my office is approximately a 5-minute walk from the South Renton Park & Ride. The building complex provides free parking.

From I-405 South: Take Exit 3/Talbot Road S. Stay in the center lane after you exit. Continue through intersection, crossing over Talbot Rd. S. You will now be on Renton Village Place. Continue straight on Renton Village Place. The Evergreen Building will be ahead, directly in front of you (next to the Roxy Cinema). I am on the 5th floor, Suite 510. From I-405 North: Take Exit 2 towards Renton/Rainier Ave. Turn right onto S. Grady Way. Turn right onto Lake Ave. The Evergreen Building is on the right just behind the Red Lion Hotel. I am on the 5th floor, Suite 510. |
Cancellation Policy
As I provide service at multiple locations in Western Washington, I ask that you contact me as soon as possible and at least 24 hours prior to your scheduled appointment.
Payment
Full Payment must be made at the time of the Initial Evaluation. My fees are not covered by insurance.
Per Federal Regulations (49 CFR Subpart O 40.289): "Payment for SAP evaluations and services is left for employers and employees to decide and may be governed by existing management-labor agreements and health care benefits.
